mpi: remove dubious MP_DEBUG cruft.

The MP_DEBUG macro controls the visibility of definitions that
are not used anywhere.

* mpi/mpi-config.h (MP_DEBUG): Removed.

* mpi/mpi.c (DIAG): Macro and surrounding section controlled
by #if MP_DEBUG removed.

* mpi/mpi.h (MP_IOFUNC): Definition controlled by MP_DEBUG
removed.
